かかりつけの先生が不在なので、空いている方なら誰でも予約を取りたいです。
「appointment」=予約。初診なら「I’m a new patient」と伝える。急ぎは「as soon as possible」。
I’m here to check in — I have a 10 o’clock appointment with Dr. Lee.
受付をお願いします。イ先生に10時の予約があります。

「check in」=受付(到着登録)。予約時間と医師名を言うと早い。受付は「reception」。
It started Monday — I’ve had a sore throat for three days now.
月曜から始まりました。3日前から喉が痛いです。

正直なところ、3日前から喉が痛くて、よくなりません。
症状は「I’ve had + 症状 + for + 期間」で事実だけ伝える。始まった時期(since Monday)も役立つ。

押すと、ちょうどここ、左下が痛いです。

指でお見せします。深く息を吸うと、ちょうどここが痛いです。
痛い所を指して「right here」。痛みは「sharp / dull / throbbing」。強さは1〜10で聞かれる。

書類に必要ですか?保険証はこちら、身分証もです。
I think my plan changed last month, but here is my insurance card for now.
先月プランが変わったと思いますが、ひとまず保険証はこちらです。
受付で「insurance card」と「ID」をほぼ必ず求められる。保険なしは「self-pay」。
It’s my first visit — do I need to fill out any forms?
初めての受診です。記入する書類はありますか?

前回から情報が変わったのですが、もう一度書類を記入する必要はありますか?
初診の問診票は「intake form」。アレルギーや服用中の薬・既往歴を書く欄がある。
I checked in a while ago — how long is the wait to be seen?
さっき受付しましたが、診てもらうまでどのくらい待ちますか?
No problem if it’s a while — could you tell me roughly how long is the wait?
少しかかっても大丈夫です。だいたいどのくらい待ちますか?
「the wait」=待ち時間。診てもらう=「to be seen」。順番は「call your name」。
My work needs proof — can I get a doctor’s note for the days I was out?
会社に証明が要ります。休んだ日の診断書をもらえますか?
Before I leave, can I get a doctor’s note to give to my school?
帰る前に、学校に出す診断書をもらえますか?
「doctor’s note / sick note」=病欠の証明。正式は「medical certificate」。発行に手数料の場合も。

これは待てません。この病院の救急室はどこですか?
